Child and Adolescent Case Management

Children receiving case management services are assigned a Case Manager who has specific knowledge about child development and local resources available for children and adolescents from age 3 through age 17.  Case managers assist individuals with developmental disabilities in obtaining services they need by assessing the individual’s needs, developing a service plan specific to the individual, advocating for the establishment of needed services, and providing direction and support to children and adolescents as they move through the service system.


Medicaid Developmental Disability Waiver Services

Developmental Disability Case Managers determine eligibility for Waiver Services.  In order to be eligible, an individual must have a diagnosis of a developmental disability and must meet functional and financial eligibility.

Frequently, referrals are made by parents or pediatricians.  However, anyone can make a referral with a parent’s consent.
