Position Number: 4275

Responsibilities: Provides intensive case management services to children and adolescents at risk of entering or currently placed in a residential facility; conducts comprehensive strength-based assessments; identifies appropriate services and implements plans of care; coordinates service delivery with providers and community resources; evaluates provider quality; provides on-site visits; and completes required documentation.

Requirements: Bachelors Degree in Human Services or a related field from an accredited college with two years of clinical experience required (can include supervised internships, practicum, or field experience providing direct services to children and adolescents with a diagnosis of mental illness; valid drivers license and safe driving record required. Fingerprints, State and FBI criminal record reports, drug test, and CANIS report will be required upon request.

ANNUAL SALARY RANGE: $31,641 - $47,461 Excellent fringe benefits.
