PACT - Support Services
Specialist
Position Number: 4242
Responsibilities: Under the supervision of the PACT Coordinator, the Support
Services Specialist assists consumers with developing functional skills and
appropriate behavior related to individual health, safety, personal care,
activities of daily living and the use of community resources and provides
consumer transportation to implement the consumers Individual Service Plan.
Provides substance abuse services, education, support, and consultation to
families; and provides crisis intervention under the clinical supervision of
staff with professional degrees. Assists consumers with weekly medication
bag/box fills. Delivers medications to the homes of consumers per
morning/evening medication runs on a rotating basis. Participates in on-call
rotation.
Requirements:
High school graduate or GED with two years
experience providing mental health services to seriously mentally
ill consumers required. Valid drivers license and safe driving
record required. Must meet Medicaid requirements for a Qualified
Para-Professional (QPP) or Qualified Mental Health Professional (QMHP).
Preferred: Associates Degree or higher in related field (social
work, psychology, psychiatric rehabilitation, sociology, counseling,
vocation rehabilitation, human services) with one year of experience
providing Mental Health Services; OR Registration as a Certified
Psychiatric Rehabilitation Provider (CPRP) with the International
Association of Psychosocial Rehabilitation (IAPSRS); OR High School
Diploma with four years experience providing services to seriously
mentally ill consumers; OR a Licensed Practical Nurse with a valid
LPN license to practice in VA. Fingerprints, State and FBI criminal
record reports, drug test, and CANIS report will be required upon
request.
ANNUAL SALARY
RANGE:
$22,788 - $34,181. Excellent fringe benefits.
